Medical Student Elective in Reproductive Health
Beth Israel Residency in Urban Family Practice

   The purpose of this experience is twofold: to provide students with exposure to abortion care in a practice where abortion care is integrated or mainstreamed into the day to day family medicine practice, and to give students an experience observing abortions at a high volume site, Planned Parenthood. They will also learn contraception counseling, options counseling, and procedures - including IUD and Implanon insertions.

I. Abortion Training

   The students on this rotation will observe first trimester abortions for one day at Planned Parenthood of New York City. They will observe the pre-abortion counseling that patients receive in this setting, as well as follow up medical abortion visits. The teaching manual that is used is the UCSF “Early Abortion Training Workbook.” A packet of reading materials is also given to the student at the beginning of the rotation.

II. Abortion Training and Care Mainstreamed

   The students will also participate in the Phillips Family Practice procedure sessions, where they will learn to counsel women on options for unintended pregnancy, to provide medication abortions, and to do follow-up visits. They will also provide contraceptive counseling, observe IUD’s and minor gynecological procedures such as endometrial biopsy, diaphragm and pessary fitting, wart treatments, bartholin cyst drainage, etc. Some of these are general procedure sessions, where other residents doing dermatology and ambulatory surgery are present doing procedures as well, the reproductive health procedures are integrated into the general family medicine procedural template and mainstreamed into the practice. Other procedure sessions are more women’s health focused.

   An additional list of readings is provided, and the text used is “Procedures in Primary Care.” The students will also participate in the Saturday Women’s Free Clinic, and the evening IUD session. The student will also see patients under the supervision of the Reproductive Health Fellow.

Resident Elective in Reproductive Health
Beth Israel Residency in Urban Family Practice

   The purpose of this experience is to provide residents with hands on experience performing procedures in the full spectrum of women’s reproductive health care. The experiences are twofold: to provide residents with exposure to health care and training at Planned Parenthood and to provide them with experience in a practice where reproductive health care is integrated or mainstreamed into the day to day family medicine residency practice..

Abortion Exposure in High Volume Site

  The residents on this rotation will receive exposure to abortion care at Planned Parenthood of New York City. The clinical training manual that is used is the UCSF “Early Abortion Training Workbook.” A packet of reading materials is also given to the resident at the beginning of the rotation. Residents will be involved in abortion services Planned Parenthood primarily as observers, seeing medication abortion, ultrasound, manual vacuum aspiration and electric vacuum aspiration.

Abortion and Reproductive Health Care Mainstreamed

  The residents will also participate in the Phillips Family Practice procedure sessions, where they will counsel women on options for unintended pregnancy, provide medical and suction abortions, and do the follow-up visits. They will also provide contraceptive counseling, insert IUD’s and Implanons and do minor gynecological procedures such as endometrial biopsy, diaphragm and pessary fitting, wart treatments, bartholin cyst drainage, etc. Since this is a general procedure session, where other residents doing dermatology and ambulatory surgery are present doing procedures as well, the reproductive health procedures are integrated into the general family medicine procedural template and mainstreamed into the practice. The resident may also be assigned one Saturday in the Women’s Health Free Clnic. An additional list of readings is provided, and the text used is “Procedures in Primary Care.” The residents will also take part in the Wednesday AM article readings and discussions, including presenting one of the articles during the rotation.

Advanced Training in Reproductive Health -
as a Fellow
 

 

Falencki Fellowship in Reproductive Health Care & Advocacy

   This one-year fellowship aims to develop leaders who will promote and teach full-spectrum women’s reproductive health care within family medicine. The fellowship is based in New York City. The fellow will participate in the following activities:

1. The fellow will spend one year as a “trainer in training,” learning to perform abortion and related procedures (IUD insertions, first-trimester sonography, medication abortion, endometrial biopsy, and manual vacuum aspiration of the uterus) and learning to teach these procedures to others. Most of this training will take place at Phillips Family Practice. The fellow will also spend 10 to 15 days at a high volume abortion site.

2. The fellow will work five patient-care sessions per week in the Beth Israel Residency Program in Urban Family Practice, seeing continuity care patients and participating in the women’s health procedure sessions. The fellow will teach residents to perform abortions, IUD insertions, and other procedures in the family medicine setting.

3. The fellow will develop teaching skills by precepting family medicine residents one session per week, learning to teach and give feedback under the supervision of the fellowship faculty. As part of the faculty development aspect of the fellowship, the fellow may take on an intern as an advisee, under the supervision of the fellowship director. The fellow will also attend faculty meetings when possible.

4. The fellow will develop teaching and leadership skills by giving presentations during the Beth Israel Residency curriculum sessions and at academic family medicine meetings. The fellow will participate in advocacy projects that promote access to reproductive health care in family medicine, with guidance from the Reproductive Health Access Project (RHAP). The fellow, in collaboration with residents and faculty, will work on the on-going projects at the Beth Israel Residency with a goal of preparing presentations for academic meetings and publications for family medicine journals.

5. The fellow will work with Medical Students for Choice (MS4C), helping them to build their local chapters and engaging residents in doing projects with the medical students. The fellow will connect MS4C students with students who volunteer at New York University (NYU) School of Medicine’s free clinic, promoting the free clinic’s expansion of reproductive health services for uninsured women.

6. The fellow will help to sustain a free reproductive health clinic, in collaboration with RHAP staff, NYU medical students, fellowship faculty, and a community advisory board. The fellow will participate in the “reproductive health team” that will perform abortions and other family planning services for uninsured women. This free clinic will serve as an abortion-training site for students and residents.
